Medically Fragile Foster Care
Our Medically Fragile Foster Care program is specially designed for children and adolescents with complex medical needs. In partnership with medical professionals and our network of dedicated caregivers, called Mentors, we can support medically-complex children in private family homes where they can receive twenty-four hour care and nurturing support.
In our Medically Fragile foster homes, each child has an individualized treatment plan and benefits from one-to-one support, medical care and social stimulation provided by their Mentor. The children we serve in our comprehensive Medically Fragile Foster Care program have a variety of medical needs ranging from mild pre-natal drug exposure to the most complex conditions, such as ventilator dependence. Our Mentors receive assistance with home modifications, medication administration and dedicated support from our team of health and human services professionals including a clinical coordinator, a nurse specialist and a pediatric medical director. Our team ensures that each child has access to the supports they need to live comfortably in the community.
